WHAT YOU NEED TO OPEN A HYPNOTHERAPY PRACTICE IN SEATTLE

The Washington State Department of Health serves as the regulatory agency for hypnotherapists in Seattle and King County. Therefore, “Hypnotherapists” and “Counselors” must register with the Washington State Department of Health (DOH).

 

Eligibility for Seattle Hypnotherapists:

In order to register your Seattle hypnosis practice with the DOH, you must:

  • Mail a credential verification form to each state or jurisdiction where you’ve been licensed, certified, or registered as a healthcare provider.

  • Pay an application fee of $155 and an annual renewal fee of $80

  • Complete four hours of HIV/AIDS training (classes are $85 in King County)

Application:

The application to register as a hypnotherapist in Washington State requires the following information:

Demographic Information:

  • Social Security Number (if you don’t have one, you’ll need to call 360-236-4700).

  • National Provider Identifier Number (NPI) from the Federal Centers for Medicare and Medicaid Services

  • Legal Name

  • Birth Date

  • Address

  • Phone, Fax, and Cell Numbers

  • Email Address

  • Other Names (if applicable)

Personal Data Questions:

  • Medical Conditions

  • Chemical Substance Use

  • Convictions, Plea Deals, Deferred Sentences, Suspended Licenses

Licenses, Certifications, and/or Registrations:

  • A list of states where licenses, certifications, and registrations are held or have previously been held

Other Information:

  • A brief description of your therapeutic technique or discipline

  • AIDS education and training attestation

  • Applicant’s attestation
